What is the Community Weight Management Service? ‘Community Weight Management Service offers clients the opportunity to reduce their weight by providing programmes that are right for them’. The service is available to people wanting to lose weight and improvement their lifestyle within the Middlesbrough, Redcar and Cleveland area. All clients are screened by a qualified weight management advisor to determine their suitability for the service as well as their programme preference. All the Weight Management programmes are facilitated by qualified weight management staff and the programmes meet NICE guidance and contribute to an evidence base. aims. The CWM service aims - limit further weight gain - achieve modest weight loss - get into a regular eating pattern - balance the types of food that they eat - become more physically active - learn new lifestyle skills - sustain their changes Criteria Inclusion criteria to access any of the service is a BMI equal or greater than 25, over 16, you are a resident of Middlesbrough, Redcar and Cleveland and ready to make lifestyle changes. CWM Programmes SHAPE UP - Friendly group support Shape-up is a structured 8 week 1 ½ hour self-help programme which aims to help people manage their weight, and gain new skills to support long-term lifestyle change. The programme focuses on a behaviour change model and is not for those wanting a quick-fix weight loss solution. The Shape-up groups should ideally consist of 8 – 15 people; this offers individuals the opportunity to work through the programme with the support of others in similar situations. L.E.A.N - A drop-in programme The LEAN (Lifestyle, energy, Activity & Nutrition) programme is a flexible drop-in service, which will support you to achieve realistic goals in order to make lifestyle changes, this programme supports you to lose weight and keep it off. The programme can be accessed at your convenience; however we do encourage you to attend the core element for the first eight weeks. Programme structure two hours; - Initial assessment and weigh in - first hour - Core educational elements and weigh in (eight elements) – half hour - Topical discussion and weigh in – half hour Once you have completed an eight week core programme, you can choose whether you wish to participate in the group sessions or just drop in to be weighed.
